COVID-19 Vaccine Information

PLEASE READ ALL OF THE INFORMATION BELOW IN IT'S ENTIRETY!

EFFECTIVE APRIL 19, 2023:

The CDC determined that individuals age 65 and older who have received a single dose of the Covid-19 Bivalent vaccine are allowed to get one additional dose at least 4 months following their initial bivalent dose.

They also determined individuals who are immunocompromised and had received the single dose of the Covid-19 Bivalent Booster are now eligible for a second Bivalent Booster dose at least two months after receiving their inital Bivalent dose.

Additionally, the monovalent Moderna and Pfizer-BioNTech Covid-19 vaccines are no longer authorized for use in the United States. The never-vaccinated who want to recive a Covid-19 vaccine, will receive a single dose of a bivalent vaccine, rather than multiple doses of the orignal monovalent vaccines.

AUGUST 26, 2023 UPDATE: We have Pfizer & Moderna Bivalent vaccines available. As of now, the new COVID-19 vaccine has not been yet signed off by the CDC & FDA, we are expecting to hear more information regarding the release of the newest vaccine by the end of September, we will notify our customers once the vaccine is available.

If you are eligible and interested in receiving the COVID-19 vaccine at Buckley’s Drug Store, please follow the steps below:

  1. Download and print our vaccine consent form located below

**Please note: this form is not editable on a computer.  It must be printed.

  • In addition to the consent form, you must include a picture of both your Medical and your Pharmacy Prescription Insurance Cards -- even if you believe we already have it. This will ensure a swifter process
    • If you do not have Medical or Pharmacy Prescription Insurance, please indicate that in your email or time of drop off
  1. Once your form is completed, you can return it to us either by scanning and emailing it to buckleysrx@gmail.com

           Drop it off in person at our pharmacy

**Please note, returning the consent form below does not  guarantee an appointment, Once you return the form with your insurance information, we will contact you if we still have vaccines available.

  1. Descargue e imprima nuestro formulario de consentimiento de vacuna que se encuentra a continuación

**Tenga en cuenta: este formulario no se puede editar en una computadora. Debe estar impreso.

  • Además del formulario de consentimiento, debe incluir fotos de ambas de su tarjeta de seguro médico y de farmacia, incluso si cree que ya las tenemos. Esto asegurará un proceso más rápido
    • Si no tiene seguro médico o de farmacia para recetas médicas, indíquelo en su correo electrónico o en la hora de entrega
